Need infobox help with recommissioning/decommissioning

The article states that the Cambria was recommissioned on 9/15/1950 and decommissioned for the second time on 9/14/70. She was also reclassified as LKA-36 in 1969. I tried to put that stuff into the infobox, but got some warnings. Will somebody please do it properly? Lou Sander ( talk) 23:03, 15 October 2015 (UTC) reply
