I removed all of the references to his first name only. Per
Wikipedia:Manual of Style (biographies), this is discouraged because "The use of the first name gives the impression that the writer knows the subject personally, which, even if true, is not relevant.". Also, in my opinion, it makes the article read like a
People Magazine fluf-piece. --

citations always needed?
Citations needed for common knowledge, including all finishing orders for Indy 500? Some of the citation requests I can understand, but other open-wheel (and other) auto racing stubs don't require this sort of citation.

Per the template documentation, cause of death should be included only when significant to the subject's notability - that bar isn't met in this case, as he's not notable for not dying in a racing accident. Heart disease is a leading cause of death for men of that age and qualifies as routine.
