I originally came to convert the bare url to a {citation} type format and found it to be {dead-link}. Found the archive and in reading it I find it to be a primary source citing a possible secondary source. I locate the secondary source (healthgrades) and I find no mention of this award for 2012. I also find that the profiles are free to facilities and are self managed by the facilities themselves who can login to do so. I removed the following from the article and placed it here as it is without secondary published sources and no other verification that the designation is recognized or anything notable. I leave the information here in the event a consensus or future editor finds sufficient source to move it back to the main article.
